This activity is used to enter information about one or more FMEA causes related to each failure. Cause of failure can be defined as an indication of a design or process weakness, the consequence of which is the failure mode. Causes are the conditions that result in a failure. Each cause can have a set of intended controls and recommendations. A control is a proposed activity that may assist in the prevention and/or detection of design or process failure or its effects. A recommendation is a suggested action for preventing a failure, reducing its severity, or improving the likelihood of its detection.
To enter detailed information about a FMEA failure cause, first open the Design FMEA or Process FMEA page, expand the record for the corresponding part in the record selector by clicking Expand and open the appropriate level. Alternatively open the FMEA Function page and select the record for the corresponding function in the record selector. Select the specific failure in the Failures section. Then, click the new command on the Causes tab, enter information about the cause, including occurance, and click Save.
This activity requires the existence of at least one structure or routing alternate with at least one FMEA function and failure.
As a result of this activity, you will have entered information about one or more causes for each failure related to the selected design or process FMEA function. You will also have entered the occurance ranking for each failure cause.
